Group Consultations Training & Implementation Pilot

The Hub Plus is offering fully funded places worth over £3,000 for up to 6 practices / PCNs to take part.

Access a comprehensive toolkit covering 60%+ of QOF pathways and key LTCs.

This programme is designed to build confidence, develop skills and support real delivery of group consultations for patients with long-term conditions.

You will:

✅ Develop a sustainable model within your team

✅ Support patient self-management and peer learning

✅ Contribute to neighbourhood working and new care models

Please note: we have funding for only 6 practices / PCNs across Derbyshire. Early expression of interest is strongly encouraged. Talk to your practice/PCN manager about this amazing offer

The Hub Plus is proud to launch its first dedicated Oliver McGowan Tier 2 Training Programme for Derbyshire General Practice teams.
