Wellbeing visits for Primary Care

Welcome to our Wellbeing Health Improvement Advisor Practice Visits page, where we provide expert guidance and personalised support to help you achieve optimal health and wellness in your practice. Our advisors are on hand to visit in person or online, offering assistance in incorporating a multitude of wellness strategies into your practice. 

Bookable appointments are available for every primary care site with a health improvement advisor to visit in person or virtually. They will be able to distribute specialist knowledge, raise awareness and promote local and national initiatives to improve wellbeing within your teams. Please note, this service is only availiable for staff and not patients within your practice.

Wellbeing services overview

Overview of all of the support available to colleagues through the Health and Wellbeing Services- covering all aspects of what's available. Including the current timetable of opportunities and workshops.

Menopause awareness

We can pop in and chat to individuals, teams / managers to offer advice and guidance around the support available to colleagues going through this phase of life, or those who are wanting to understand more about it and support those around them.

Stress release

Our stress awareness talk will educate you on what causes stress and the ways we can overcome it. We will take you through some useful tools you can use to help overcome the symptoms of stress.

Physical activity

Our Health Improvement Advisors can pop into a team meeting to tell you about the wide range of physical activity opportunities that we have on offer. Everything from Seated Exercises classes, beginners exercise, through to sports and social activities and classes such as legs, bums and tums and yoga.

How to book a wellbeing visit

1. Complete our short expression of interest form

For any wellbeing visits, no matter how big or small, we kindly ask that all requests go through the same expression of interest form. If there is a workshop not listed on the form which you would like to enquire about, please email ddlmc.thehubplus@nhs.net.

We ask that any requests are submitted at least 4-6 weeks in advance of your requested date. This will allow us to coordinate the request appropriately and assign relevant team members to attend your requested workshops.

 

2. What happens next?

Once you have completed an expression of inteterest form, a member of the team from The Hub Plus should be in contact with you within 7 working days from your request. Depending on the size of the request (over 20+ staff expected to attend), we may ask to meet with you via teams/ telephone call to discuss your request. Whilst we coordinate the HIA'd diaries and the small details in the background, please do feel free to get in contact if you have any other questions. 

 

3. Enjoy your visit!

Once we have coordinated the HIA's schedules, we will send you an overview of your booking request along with any other necessary details to ensure the session runs smoothly. This overview will outline each party's responsibilities to guarantee a successful session. Our HIAs will attend and deliver the selected workshops, aiming for your staff to have a fantastic and engaging wellbeing experience. Feedback will be requested from the individual who booked the session, and we encourage sharing this feedback with the entire team involved. This helps us continue to provide exceptional wellbeing services to primary care in Derbyshire.

Important information

Please note that the Wellbeing Team primarily operates during standard working hours on Wednesdays. Visits on other days are subject to team availability and may be adjusted accordingly. If your visit requires travel over long distances, please ensure to communicate this in advance, as it may affect the scheduling and logistics of the visit. We aim to accommodate all requests, but flexibility may be necessary depending on the availability of the team, travel considerations, and other commitments.
